Description:
Mad Sybil is located in The Otherworld inside a ruined tower (loc 33000 26500). She's a level 44 mob and highly aggressive, but cannot leave her tower right off.
Once you hit this mob, she appears as a ghost and you are assaulted by a seemingly random number of lower level Voice of Madness. You must kill each mob to make Mad Sybil able to be attacked again. During this assault, Mad Sybil will stay out of the fight until the last Voice of Madness drops.
Even at level 50, you may require some assistance. Mad Sybil drops a series of unique items at complete random. If there is a specific one you are after, you may have kill her repeatedly to get what you need. From my experience, she drops no more than two of her items, if she drops anything at all.
Respawn seems that of a regular mob, taking anywhere from immediately, to several minutes.
