No.... quality will NEVER decrease. It's condition that decreases when the item is used, and especially if the char is too low for the item. When repaired, condition will go up to 100% again, and durability will decrease.
Think of the quality as the material used for creating the item's quality. How pure was the ore when crafting this sword? How crooked was the stick this staff was made out of? And so forth....

If it has 100% quality does it never have to be repaired or is it unbreakable?

100% quality is just the starting quality ... when you are in battle, the quality of the item will decrease ... you can put it back to 100% by repairing it (or find a smith to repair it) ... which will decrease the durability of the item ...
