You need to find a Legendary Grandmaster Spellcrafter and Alchemist in order to make the head, and in the end it costs about a plat. You can find all of the componants needed to make the parts that are imbued into the head at any spellcraft/alchemy merchant and the spellcraft/alchemy merchant in the Meath Market, except for the cleansed daemon diamond seal and Balor's Remains. To get the cleansed diamond seal, take any diamond seal from darkness falls and hand it to the taxidemrist, Shaughnessy, in the Meath Market, and he will "cleanse" it. Then give it to your spellcrafter, along with a bewitched hammer which you can buy at the spellcraft merchant in the Meath Market and she can crush it. The Balor's Remains drop off of none other than Balor himself and, once all of these componants are assembled and given to the taxidermist, he will give you Balor's Wall Trophy. |

The name of the taxidermist is Shaughnessy. He does not have a title so you wouldn't know which NPC to turn the diamond seal into unless you tried all of them ;)
