I just did this quest with my inf and it seems there has been an update on the gem stats. The gem i got is lvl 17, it has 2 stealth, 2 critical strike, 1% slash resist, and 20 hits.

im an 21 inf now. is there anything to do and get the 15 and 20 epics?
i think my mystake is that i didnt follow the lady back to pk during the 7lvl quest. but the NPC at wd toldme that i didnt have too

If by following the NPC ya mean you never went to WD to get your item, that would be your problem. But another possible reason is that you didn't do your level 11 epic. If so, then there's your problem.
