This item is a uncommon drop from the howling knifemen. It drops once in your average fifty kills from the howling knifemen. Ironically, this drops more often than the gold or silver from my experience. It is a very nice (and fast!) dagger for any assassin out there.

I just got one tonight off of a howling maiden. Tis very real.
.. quite real. Most fools who scream "fake" are some low life newbies still hunting plague spiders in the newbie fields and have no idea about the lvl 30+ mobs and loot drops. So just ignore them..
I've seen this weapon around, to those people who have been shaken by the previous poster's admitted "It's a fake" forgery post :)
